Certificate of Insurance Request Letter: Templates for Every Situation

Copy-paste COI request templates — new vendor, tenant move-in, renewal, and correction — written to get a compliant certificate in one round instead of five.

Diamond Flooring starts next week. Travis finds that the general liability limit needs an increase and the additional insured is not listed, emails the vendor, verifies coverage with the insurance broker, and updates the new COI in the system before work begins.

A COI request letter has one job: to get back a compliant certificate on the first try. Most requests fail at that job because they ask for "your COI" — which reliably produces a bare ACORD 25 with nothing attached, followed by weeks of correction rounds. The fix is specificity: name the entities, the endorsements, and the phrase "endorsement pages attached," and you've moved the correction rounds into the first email.

Four templates below, one per situation. Adapt the bracketed parts; keep the specificity.

1. New vendor request

Subject: Insurance certificate needed before your first work order — [Company]

Hi [name],

Welcome aboard — before we can issue your first work order, we need a certificate of insurance per your service agreement. To save your insurance agent a second round, the certificate needs to show:

• Certificate holder: [exact legal entity, address] • Additional insureds, by endorsement with the endorsement pages attached: [owner entity] and [management company] • General liability of at least [$1,000,000] per occurrence / [$2,000,000] aggregate, primary and noncontributory • [Auto liability of $1,000,000 CSL / umbrella of $X / workers' compensation at statutory limits, per your agreement] • Waiver of subrogation in our favor (GL and workers' comp)

A note in the certificate's description box isn't sufficient for the additional insured or waiver items — we need the endorsement forms themselves attached. Your agent can send everything directly to [email]. Happy to jump on a call with them if that's faster.

2. Tenant move-in request

Subject: Insurance certificate before move-in — [Suite/space], [Property]

Hi [name],

Ahead of your move-in on [date], we need the certificate of insurance required under Section [X] of your lease. Your insurance broker will need these specifics:

• Certificate holder: [landlord entity, address] • Additional insureds by endorsement (forms attached): [landlord entity] and [management company] — exactly as named in the lease • Commercial general liability of [$X] per occurrence / [$X] aggregate, primary and noncontributory • Property coverage for your improvements and contents at replacement cost • [Other lease-specific items]

Feel free to forward this email to your broker — it has everything they need. We'll need the certificate on file before keys are released.

3. Renewal request (send 30 days before expiration)

Subject: Insurance renewal certificate — [Company], policy expiring [date]

Hi [name],

Our records show your [GL/umbrella/auto] policy expires [date]. Please have your agent send the renewal certificate before then, with the same items as your current one: additional insured endorsements attached for [entities], primary and noncontributory, waiver of subrogation, limits per your [lease/agreement].

One ask: renewals are where endorsements most often get dropped on the new policy — please flag to your agent that we'll be checking the attachments, not just the dates. [Work orders/payments] continue uninterrupted as long as the renewal is on file by [date].

4. Correction request (send to the agent directly, vendor cc'd)

Subject: Corrected COI needed — [Vendor], [property/project]

Hi [agent name],

Thanks for the certificate for [vendor]. Two items before we can mark them compliant:

  1. [Specific gap — e.g., "The additional insured endorsement isn't attached; the description box references it, but we need the form itself (CG 20 10 and CG 20 37 or equivalents) naming [exact entities]."]
  2. [Specific gap — e.g., "The umbrella shows expired 3/1; we need current dates on that line."]

Everything else checks out. Could you reissue with those corrected? [Vendor] has [a work order/pay app/move-in] waiting on this, so this week would be ideal.

Why these work

Three patterns run through all four. Route to the producer: the agent issues certificates, so every hop through the vendor adds days — get the request in front of the agent verbatim (that's why "forward this to your broker" appears in each). Name the consequence and the date: work orders, keys, pay apps — requests tied to a moment of leverage close; favors float (more on timing). Specify the attachment rule: the single sentence about endorsement pages attached prevents the most common correction round in existence — the description-box non-endorsement.

FAQ

Who should I send a COI request to? The vendor or tenant first (they own the relationship with their agent), written so it can be forwarded verbatim. For corrections, go agent-direct with the counterparty cc'd.

How long should I give a vendor to provide a COI? A bare certificate: 3–5 business days. Anything requiring endorsement changes: 2–3 weeks, because those route through underwriting. Which is why renewals get requested at −30 days.

Can I require a COI before signing a contract? You can require evidence of insurability before signing and the compliant certificate before work/occupancy — the sequence most agreements use, since the endorsements are typically triggered by the signed written contract itself.
